Mandatory Forms - Tackle Football

In order for your player(s) to participate in the 2024 Fall Tackle season with 703 United, VAYFA and AYF, the following documents must be completed, turned into the Team Coordinator or Head Coach and certified before your player will be eligible to participate in practices and/or games:

• Vaild Official Form of ID such as a State ID, Passport or Military ID: 703 United strongly encourages that a vaild State ID such as a Walker's ID is obtained from the DMV as IDs will be collected at the beginning of the season and remain with the Team Coordinator until the season is over.
• Copy of Birth Certificate: Copy must be legible and complete.
• Copy of Final 2024 Report Card:  This must show the final grades for the year as well as the grade the individual is to attend in the 2023-2024 School Year.  If a student was homeschooled for the 2023-2024 school year, you must provide the final evaluation of their homeschooling program and complete this Homeschooling document. 
• VHSL Physical Form or Medical Clearance Form dated in 2024:  703 United strongly encourages you to get both forms completed. Both forms must be signed and dated by a certified Physican.  Physical form must clearly annotate the player is Cleared without any Restrictions for any athletics and/or tackle football.
• Participation, Tracking and ID Card Form:  Complete the top section of page 1 as must as possible and all of page 2. Pay careful to each section that needs to be initialed; there should be a total of 5 initials from parents and 1 initial from the plater. 
• Image Release - Minor:  Must be completed and signed by the Parent/Guardian.
• Waiver of Liability - Minor: Must be completed and signed by the Parent/Guardian and Player
• Emergency Medical Treatment, Consent and Information: Must be completed and signed by the Parent/Guardian.
• Concussion Statement and Acknowledgement Form: Must be completed and signed by the Parent/Guardian and Player
• Code of Conduct- AYF: Must be completed and signed by the Parent/Guardian.
• Image Release - Adult: Must be completed and signed by at least one Parent/Guardian for each player. If there are other individuals that will be volunteering to help with field activities such as holding chains, field marshall, etc, they must complete and sign as well.
• Waiver of Liability - Adult: Must be completed and signed by at least one Parent/Guardian for each player. If there are other individuals that will be volunteering to help with field activites such as holding chains, field marshall, etc, they must complete and sign as well.

FAILURE TO OBTAIN, COMPLETE OR TURN IN THESE DOCUMENTS WILL RESULT IN YOUR PLAYER'S INELIGIBLITY FOR GAMES AND PRACTICES.
